- Sleep collapse — either insomnia for more than a week, or sleeping 12+ hours and still feeling exhausted.
- Suicidal ideation — any thought, even fleeting, that the world would be easier without you in it. This one is non-negotiable: call a crisis line today.
These are not signs that you are weak or unsuited to academic work. They are signs that your nervous system is asking for help.

Who to Call, in Order
The escalation path looks intimidating only because nobody draws it for you. Here it is, low to high:
- Lecturer or supervisor — for an extension, a reduced scope, or simply naming what is happening.
- Student counselling (Studienberatung / psychologische Beratung) — free, confidential, trained to triage. They will tell you whether you need a GP or a therapist next.
- GP / Hausarzt — can issue a Krankschreibung the same day and refer you onward.
- Psychotherapist — ask for an Akutsprechstunde via 116 117 in Germany. Acute slots exist exactly for crises like this one.
- Crisis line or emergency room — if you cannot keep yourself safe between now and your next appointment, this is the right level of care.

Sick Leave Does Not Ruin Your Degree
This is the single most common reason students avoid asking for help, and it is wrong. At German universities a medical certificate (Attest) submitted to the examination office pauses the Bearbeitungszeit. The clock literally stops. The failed-attempt counter does not advance. When you are well enough to continue, the remaining writing time resumes from where it left off.
Most examination offices will accept a standard sick note from your GP for short pauses. For longer absences they may ask for a more detailed certificate from a specialist; the counselling service can tell you which form your faculty expects and how to submit it without disclosing your diagnosis to your supervisor.
Your final degree certificate shows your grade and your topic. It does not show the pause, the certificate, or the reason for it. Future employers see a passed thesis. They do not see the route you took to get there. Plenty of strong theses, and plenty of strong careers, have been built on top of a paused semester.
